The following immunizations and immunization certificates are required for entrance to Shiloh Hills Christian School.

For students entering Pre-K, Kindergarten, or school for the first time in Georgia:
All children entering school in Georgia for the first time (at any age up to 19 years) must be age-appropriately immunized with all of the required vaccines (DTP, DTaP, DT, or Td; Hepatitis B; Hib; OPV or IPV; Measles, Mumps and Rubella or MMR; and Varicella.)  Two doses of the Varicella vaccine have been added to the list of required vaccines for attendance.  Hepatitis B vaccine is now required for a child at the time of entry to school.  HEP A is required for children born on or after January 1, 2006.  Also, two doses of a measles-containing vaccine, two doses of mumps vaccine, and two doses of rubella vaccine are now required at the time of entry to school.  A new Certificate of Immunization (Form 3231) must be submitted to the school office prior to a student’s starting school.  To be valid, a certificate must have either the “Date of Expiration” or the “Complete for School Attendance” box marked.  Tdap and Meningococcal conjugate vaccines are required for students entering 8th through 12th grades effective 7-1-14.

For students already enrolled in a Georgia school: Students who are already enrolled in a school in Georgia and who are transferring to SHCS must submit a Georgia Form 3231 prior to admission to school.

For students entering 6th grade: Students entering 6th grade must provide proof of having received a second dose of measles-containing vaccine (MMR), two doses of mumps vaccine and one dose of Rubella vaccine or laboratory proof of immunity against measles, mumps, and Rubella.  Student must also have proof of 2 doses of Varicella vaccine or documentation of having had chickenpox.  (This has to be documented on Form 3231 and signed by a physician.)

For students entering 7th grade and New Entrants to Georgia schools in grades 8th-12th grades: Students are required to have Tdap (Tetanus, Diphtheria and Pertussis) and Meningococcal conjugate vaccines before entering school for the 2014-2015 school year.

For students who do not receive immunizations: If parents elect to exempt their children from immunizations, they must submit one of the following:

  1. A letter from their pastor stating religious objections or a handwritten letter from the parent.  (These must be notarized.)  Once one of these is submitted, it does not have to be submitted again.
  2. A letter from their doctor stating the reason for medical exemption from immunization.  The medical exemption must be submitted annually.
